The cylinders described in this page are operated by a vacuum, while the creation of a vacuum in the cylinder front chamber, connected to the piston, overcomes the opposing spring force. In addition, the piston is pushed by the air at an atmospheric pressure that can get the rear chamber to pass through the hollow stem.
The greater the pressure differential is, the larger the piston thrust will be. Note that the stem returns to position in two ways. First of all, it can be done by preventing atmospheric air from entering using the stem hole, in which case atmospheric pressure forces and thrust spring prevail on the stem. Another option is exclusion of the vacuum, in which case the atmospheric pressure will eventually be restored.
